1. Looks like a jiffy cap. Must be he was out of wing nut plugs. Next time make a donut out of foamboard to keep your pipe centered in the sleeve and keep conrete out. Use a riser clamp to hold it up, with beam clamps on the 6″ to keep it from turning/twisting.
