FYI- yesterday’s irregularities were from power outage

DF1 – 25/06/2014 **

Netrouting DF1 suffered from a brief power outage on June 25, 2014.

OUTAGE TYPE: Power phase failure which lead to outage on
protected power grid

IMPACT: Service interruption

LOCATION: Spijkenisse, NL

OUTAGE START TIME: 06/25/2014 16:09:05 CET

OUTAGE END TIME: 06/25/2014 16:09:50 CET

REASON FOR OUTAGE: 

Due to a yet to be determined cause, the L1 phase on our protected grid
failed yesterday. Our UPS switched over to its batteries to deliver power.
While having our genset running, two engineers on-site troubleshooted the
installation with our supplier via phone, who recommended to put the UPS in
external bypass and have net power synchronize with our diesel generator for
redundancy and restoration of the L1 phase, while we further investigated the
L1 phase failure.

With rapidly draining batteries a decision had to be made which due to
miscommunication lead to the wrong circuit being cut off. Steps were almost
instantly reversed but for several seconds Zone A was without power. Proper
procedures were followed during this process, having a second man present to
confirm instructions are being followed as described. Unfortunately the initial
outage is caused by human failure, in no way did our UPS fail during this
process.
